Member Agreement

The strength of Eye Recommend (Doctors Eye Care Co-op Ltd.) depends on its members’ strength and shared vision for full-scope, independent optometry ownership. Members commit to the following list to share the common values of Eye Recommend, aiding in growth in a unified direction. Having reviewed the Membership Eligibility, you agree to the following:

• I am committing to practice and support Independent Optometry through independent business ownership and operational decision-making.
• I am committing to provide data to Eye Recommend for comprehensive benchmarking tools to strengthen the cooperative.
• I am committing to progressive optometry and incorporating new technology, products, and services to improve patient eye health.
• Supporting the Eye Recommend approved suppliers through their products and services.
• I am committing to the highest standards of professional ethics and patient welfare.
• To be a member in good standing of a Provincial Optometric Association and encourage associates at my practice to be members.
• Confirm that I am not part of the panel for any Managed Care groups with exam and product discounts as part of their agreement.
• Use any Eye Recommend trademarks, logos or other materials only as outlined and permitted by Eye Recommend at their sole discretion.
• Allow Eye Recommend to use my participation in activities to promote the organization with suppliers, social media, or the industry.
• Pay all Eye Recommend statements by the due dates and agree to interest charges and further actions resulting from non-compliance.
• Purchasing all products and services involving Eye Recommend approved suppliers through the Eye Recommend billing system.​
• Providing written notice of my intent to resign or retire my Eye Recommend membership at least 30 days before the effective date.
• To update any critical information changes regarding the practice or membership.

I acknowledge the principles and values of Eye Recommend and agree to practice in a way that encompasses all the above. I understand that membership is a privilege with responsibilities and that my membership is dependent upon my ability to perform these responsibilities. The Eye Recommend Board of Directors can terminate, suspend, or update my membership status under approved policies.
